Does anyone make these? If so who?

I've been using JnJ 5-chime FWD "Amtrak" horns as stand-ins, but I'm not sure if they are available anymore. Someone mentioned a plastic Atlas part but I think the hoarders have snatched them all up - I could never find them listed on their site. The new BLMA brass 5-chime units look better in person than they do in the advertising images!

Not all 5 chimes are created equal. BLMA is a Leslie, while the Atlas is a Nathan. Nathans are noticeably smaller than Leslies as well as the pattern of their frames too.

If I remember correctly, C&O F7s had the Nathan M5 horns. The Western Maryland F7s had dual Nathan M5s. I'm not positive, but I think the WM FA2s had Nathan M5s, too.

EDIT: I just went to the BLMA site - looks like the Leslie S5T will need to be the stand-in for the Nathan M5 horns...

This is about as good as a photo of the horn that you will find...

and the horn, to me, looks pretty close to the Leslie S5T.

Interesting, really, that all the "chimes" are in one direction but I would guess that this is because the F7As weren't expected to lead a cut of cars in reverse.

In looking at the Leslie S5T it appears to be a good horn for most Southern engines as well.

Yes, those (on the C&O F7) look like the Nathan M5.
That is also what I need for my NKP Alco PAs.

I see that JnJ Trains still has 5 chime Leslie and Nathans:

Also listed on Walthers web site.

Its not correct for a Nathan M series. It's more like the Nathan K series.

Nathan M's are definitely made...I have a few that I had picked up from a random individual at a trainshow that had a slew brass parts years and years ago, when the second-hand market was ripe. I wouldnt be surprised if BLMA comes out with a Nathan 5 chime in a year or so based on their current offerings.
