When (and why) did they remove the nose door and second headlight?

The head lights were gone after the locomotives were rebuilt with GP-38-2 guts in 1994. I believe the nose doors were welded shut in 2003 for crew safety after the Centennial, UP 6936 was involved in a grade crossing accident on November 30, 2000 at Vacherie, La.

After going through all the photos and drawings in my books, I have concluded that most if not all the E-9s in UP's fleet came with only the upper headlight while the E-8s had upper and lower headlights.
