I have train orders from a few of those locations he mentioned. Probably none with his signature. I may look, just for fun. On the PRR, a "Form A" was a clearance card. A "Form K" was a permissive card. This is what an actual "hoop" looks like, as he described them being thrown back. The later 'fork' style was not thrown back, and used the break away string instead. To the end, however, they still referenced the fork type as a "hoop".
